Transition Care Program
As an older person you might need extra time to recover after a hospital stay, and to work out the best place for you to live longer term.
The Transition Care Program (TCP) is a 5-12 week program to help get you back to life and to make an informed decision about what happens next. The care is available in hospital and at home, and often happens as a combination of both.
After a hospital stay it can take a while to get used to doing things independently again – like cooking, hanging out the washing or even collecting the mail. We have simulated facilities on site so you can practice with support at hand.
On the ward we have dedicated rooms for TCP patients. While you’re here we’ll support you with tailored assessment and rehabilitation support from your personal healthcare team. You’ll have access to the gym plus indoor and outdoor facilities.
